RESUMO

We show, for the first time, radio measurements of the depth of shower maximum (X_{max}) of air showers induced by cosmic rays that are compared to measurements of the established fluorescence method at the same location. Using measurements at the Pierre Auger Observatory we show full compatibility between our radio and the previously published fluorescence dataset, and between a subset of air showers observed simultaneously with both radio and fluorescence techniques, a measurement setup unique to the Pierre Auger Observatory. Furthermore, we show radio X_{max} resolution as a function of energy and demonstrate the ability to make competitive high-resolution X_{max} measurements with even a sparse radio array. With this, we show that the radio technique is capable of cosmic-ray mass composition studies, both at Auger and at other experiments.

RESUMO

Instantons, which are nonperturbative solutions to Yang-Mills equations, provide a signal for the occurrence of quantum tunneling between distinct classes of vacua. They can give rise to decays of particles otherwise forbidden. Using data collected at the Pierre Auger Observatory, we search for signatures of such instanton-induced processes that would be suggestive of super-heavy particles decaying in the Galactic halo. These particles could have been produced during the post-inflationary epoch and match the relic abundance of dark matter inferred today. The nonobservation of the signatures searched for allows us to derive a bound on the reduced coupling constant of gauge interactions in the dark sector: α_{X}≲0.09, for 10^{9}≲M_{X}/GeV<10^{19}. Conversely, we obtain that, for instance, a reduced coupling constant α_{X}=0.09 excludes masses M_{X}≳3×10^{13} GeV. In the context of dark matter production from gravitational interactions alone, we illustrate how these bounds are complementary to those obtained on the Hubble rate at the end of inflation from the nonobservation of tensor modes in the cosmological microwave background.

INTRODUCCIóN: La cefalea es el motivo de consulta neurológico más prevalente en los distintos niveles asistenciales, donde la anamnesis y exploración son primordiales para realizar un diagnóstico y tratamiento adecuados. Con la intención de unificar la atención de esta patología, el Grupo de Estudio de Cefalea de la Sociedad Española de Neurología (GECSEN) ha decidido elaborar unas recomendaciones consensuadas para mejorar y garantizar una adecuada asistencia en Atención Primaria, Urgencias y Neurología. METODOLOGíA: El documento es práctico, sigue el orden de la dinámica de actuación durante una consulta: anamnesis, escalas que cuantifican el impacto y la discapacidad y exploración. Además, finaliza con pautas para realizar un seguimiento adecuado y un manejo de las expectativas del paciente con el tratamiento pautado. CONCLUSIONES: Esperamos ofrecer una herramienta que mejore la atención al paciente con cefalea para garantizar una asistencia adecuada y homogénea a nivel nacional.

Introduction: Headache is the most common neurological complaint at the different levels of the healthcare system, and clinical history and physical examination are essential in the diagnosis and treatment of these patients. With the objective of unifying the care given to patients with headache, the Spanish Society of Neurology's Headache Study Group (GECSEN) has decided to establish a series of consensus recommendations to improve and guarantee adequate care in primary care, emergency services, and neurology departments. Methods: With the aim of creating a practical document, the recommendations follow the dynamics of a medical consultation: clinical history, physical examination, and scales quantifying headache impact and disability. In addition, we provide recommendations for follow-up and managing patients’ expectations of the treatment. Conclusions: With this tool, we aim to improve the care given to patients with headache in order to guarantee adequate, homogeneous care across Spain. (AU)

BACKGROUND AND OBJECTIVE: Patients with heart failure are classified into three phenotypes based on left ventricular ejection fraction. This work aimed to compare the clinical profile, treatment, prognosis, and causes of death of patients with heart failure and reduced (<40%, HF-rEF), preserved (≥50%, HF-pEF), or mid-range (40-49%, HF-mrEF) left ventricular ejection fraction. METHODS: An analysis was conducted on the clinical data included in a prospective registry of patients with heart failure who were referred to a specific Cardiology unit from 2010 to 2019. RESULTS: A total of 1404 patients with HF-rEF, 239 patients with HF-mrEF, and 266 patients with HF-pEF were analyzed. Significant differences were observed among the groups in regard to several clinical characteristics and the frequency of prescription of neurohormonal blocking drugs. A multivariate Cox regression revealed an increased risk of all-cause mortality in patients with HF-pEF (hazard ratio 1.36; 95% confidence interval 1.03-1.80; p = 0.028) and patients with HF-mrEF (hazard ratio 1.36; 95% confidence interval 1.03-1.78; p = 0.029) as compared to patients with HF-rEF. Heart failure was the most frequent cause of death in the three subgroups. A higher relative weight of sudden death as a cause of death was observed among patients with HF-rEF while the relative weight of non-cardiovascular causes of death was higher among patients with HF-pEF and HF-mrEF. CONCLUSIONS: This study confirms the existence of significant differences among patients with HF-rEF, HF-mrEF, and HF-pEF with regard to their clinical profile, therapeutic management, prognosis, and causes of death.

Antecedente y objetivo: Los pacientes con insuficiencia cardíaca se caracterizan en 3 fenotipos en función de su fracción de eyección ventricular izquierda. El propósito de este estudio fue comparar el perfil clínico, el tratamiento, el pronóstico y las causas de muerte de los pacientes con insuficiencia cardíaca y fracción de eyección ventricular izquierda reducida (<40%, IC-FEr), preservada (≥50%, IC-FEp) o en rango medio (40-49%, IC-FErm). Metodología: Análisis de la información clínica recogida en un registro prospectivo de pacientes con insuficiencia cardíaca remitidos a una consulta monográfica de Cardiología entre 2010 y 2019. Resultados: Se estudiaron 1.404 pacientes con IC-FEr, 239 pacientes con IC-FErm y 266 pacientes con IC-FEp. Se observaron diferencias significativas entre los 3 grupos en relación con diversas características clínicas, y en cuanto a la tasa de prescripción de fármacos moduladores de la respuesta neurohormonal. La regresión de Cox multivariante reveló un incremento del riesgo de muerte por cualquier causa en los pacientes con IC-FEp (hazard-ratio 1,36; intervalo de confianza al 95% 1,03-1,80; p=0,028) e IC-FErm (hazard-ratio 1,36; intervalo de confianza al 95% 1,03-1,78; p=0,029) en comparación con los pacientes con IC-FEr. La insuficiencia cardíaca fue la causa más frecuente de muerte en los 3 grupos; se observó un mayor peso relativo de la muerte súbita en los pacientes con IC-FEr, mientras que las causas no cardiovasculares de muerte tuvieron un peso relativo mayor en los pacientes con IC-FEp e IC-FErm. Conclusiones: El estudio confirma la existencia de diferencias significativas en el perfil clínico, manejo terapéutico, pronóstico y causas de muerte de los pacientes con IC-FEr, IC-FErm e IC-FEp (AU)

Antecedentes y objetivos: La estimación del riesgo cardiovascular en personas mayores de 70 años es problemática. La mayoría de las escalas se han creado basándose en cohortes de personas de mediana edad, con una representación insuficiente de los adultos de más edad. El poder predictivo de los factores de riesgo cardiovascular clásicos disminuye con la edad. El objetivo de este estudio es desarrollar una escala específica para estimar el riesgo cardiovascular de la población anciana española. Métodos: Este estudio se realizó en una cohorte poblacional establecida en 1995. Marco: 3 zonas geográficas de España (Madrid, Ávila y Lugo). Participantes: 3.729 personas mayores de 64 años sin enfermedades cardiovasculares (ECV) al inicio del seguimiento. Mediciones: se investigaron anualmente las sospechas de ECV mortal y no mortal (cardiopatía coronaria e ictus) y se confirmaron usando los criterios del proyecto MONICA de la OMS. Se siguió a todos los participantes hasta que apareció el primer episodio de ECV, hasta su muerte o hasta el 31 de diciembre de 2015. Resultados: La edad fue el factor predictivo más potente de ECV a los 10 años en ambos sexos. Las variables asociadas con ECV en los varones fueron el tratamiento de la hipertensión arterial (HR: 1,35; IC 95%: 1,067-1,710), la diabetes (HR: 1,359; IC 95%: 0,997-1,852) y el tabaquismo (HR: 1,207; IC 95%: 0,945-1,541), y en las mujeres, el tabaquismo (HR: 1,881; IC 95%: 1,356-2,609) y la diabetes (HR: 1,285; IC 95%: 0,967-1,707). El colesterol total no aumentó el riesgo de ECV ni en varones ni en mujeres. Sin embargo, las concentraciones de colesterol total>200mg/dL se asociaron inversamente al riesgo de ECV a los 10 años, tanto en varones como en mujeres. conclusiones: La ECV total a los 10 años aumenta significativamente en los varones españoles de edad avanzada con la edad, la diabetes y el tratamiento antihipertensivo, y en las mujeres con la diabetes y el tabaquismo (AU)

Total cholesterol did not increase the risk of CVD in men or women. However, total cholesterol levels>200mg/dL were inversely associated with 10-year risk of CVD in men and women. Conclusions: In elderly Spanish men, total CVD at 10 years is significantly increased by age, diabetes, and antihypertensive treatment and in elderly Spanish women by diabetes and smoking. BACKGROUND: The general objective of the study was to reflect on the key factors to advance in optimal models of care for Heart Failure (HF) and specifically, on the macromanagement elements most necessary for the development of comprehensive HF management models. MATERIAL AND METHODS: An Advisory Committee, composed of 15 experts and a multidisciplinary group of 31 additional experts, was appointed, together forming a Delphi panel of 46 experts. Based on a systematic bibliographic review and the analysis of the care course of the patient with HF, an initial battery of key factors for the development of HF care models was identified by the Advisory Committee. This proposal was adjusted and prioritized by the Delphi panel applying Delphi Rand/UCLA methodology. RESULTS: After two Delphi rounds, 75 key factors grouped into 7 challenges were defined. In the first of the challenges, related to the development of HF management models, 16 key factors were identified, 7 of which were valued as high priority and related to the establishment of common objectives, resources for the continuity of care and improving the measurement of health outcomes. CONCLUSIONS: The definition of management elements at the macro level was considered a priority to advance in the development of optimal models of assistance to HF.

RESUMO

We present the first measurement of the fluctuations in the number of muons in extensive air showers produced by ultrahigh energy cosmic rays. We find that the measured fluctuations are in good agreement with predictions from air shower simulations. This observation provides new insights into the origin of the previously reported deficit of muons in air shower simulations and constrains models of hadronic interactions at ultrahigh energies. Our measurement is compatible with the muon deficit originating from small deviations in the predictions from hadronic interaction models of particle production that accumulate as the showers develop.

We report a measurement of the energy spectrum of cosmic rays above 2.5×10^{18} eV based on 215 030 events. New results are presented: at about 1.3×10^{19} eV, the spectral index changes from 2.51±0.03(stat)±0.05(syst) to 3.05±0.05(stat)±0.10(syst), evolving to 5.1±0.3(stat)±0.1(syst) beyond 5×10^{19} eV, while no significant dependence of spectral features on the declination is seen in the accessible range. These features of the spectrum can be reproduced in models with energy-dependent mass composition. The energy density in cosmic rays above 5×10^{18} eV is [5.66±0.03(stat)±1.40(syst)]×10^{53} erg Mpc^{-3}.

The pedigree file of the Boer and Nubian goat breeds in Mexico was constructed using the national database provided by the Asociación Mexicana de Criadores de Ganado Caprino de Registro. Field technicians routinely updated the goat national database by recording information from flocks participating in the performance-recording system. Information on animal identification number, parents, birth date, sex, breed, and farm of origin were used to undertake pedigree analyses using the ENDOG program (version 4.8). This paper presents a pedigree data file, tables and figures of characteristics of pedigree data, pedigree analyses, pedigree integrity, effective population size and genetic conservation index. The data can be used to estimate other population parameters, to monitor the genetic diversity of the Boer and Nubian goat breeds in Mexico, and also to design balanced breeding programs, maintaining genetic variation at reasonable levels and maximizing genetic progress in these populations.

We have used gas chromatographic retention data together with other data to obtain Abraham descriptors for 30 terpene esters. These include the air-water partition coefficient, as log Kw, for which no experimental values are available for any terpene ester. The other descriptors are the ester dipolarity, S, the hydrogen bond basicity, B, (the ester hydrogen bond acidity is zero for the esters studied), and L the logarithm of the air-hexadecane partition coefficient. Both S and B are larger than those for simple aliphatic esters, as expected from the terpene ester structures that include ring systems and ethylenic double bonds. These descriptors can then be used to obtain a large number of physicochemical and environmental properties of terpene esters. We have analyzed experimental results on human odor detection thresholds and have constructed another equation for the calculation of these thresholds, to go with a previous equation that we have reported. Then the descriptors for terpene esters can be used to estimate the important odor detection thresholds.

Data on the description of growth of female Boer goats from the Mexican national breeding flock are presented. Goat meat is highly appreciated for the preparation of traditional dishes of Mexican cuisine, and its demand is on the rise. Boer goats are of relatively recent arrival in Mexico and the size of the performance-recorded flock has been increasing steadily in the last ten years. Repeated measures of body weight at different ages from birth to adulthood of Boer goats are scarce. When available, such data can be used to describe the growth pattern and the meat production potential of goat meat breeds such as the Boer. This paper presents data on estimators of growth curve parameters, plots of average predicted growth curves, plots of residuals on age, and data on goodness of fit statistics of ten non-linear functions fitted to describe the growth curve of Boer goats.

Although the metastasic breast cancer is still an incurable disease, recent advances have increased significantly the time to progression and the overall survival. However, too much information has been produced in the last 2 years, so a well-based guideline is a valuable document in treatment decision making. The SEOM guidelines are intended to make evidence-based recommendations on how to manage patients with advanced and recurrent breast cancer to achieve the best patient outcomes based on a rational use of the currently available therapies. To assign a level of certainty and a grade of recommendation the United States Preventive Services Task Force guidelines methodology was selected as reference.

Experiments and numerical simulations are described that develop quantitative understanding of atomic motion near the surfaces of nanoscopic photonic crystal waveguides (PCWs). Ultracold atoms are delivered from a moving optical lattice into the PCW. Synchronous with the moving lattice, transmission spectra for a guided-mode probe field are recorded as functions of lattice transport time and frequency detuning of the probe beam. By way of measurements such as these, we have been able to validate quantitatively our numerical simulations, which are based upon detailed understanding of atomic trajectories that pass around and through nanoscopic regions of the PCW under the influence of optical and surface forces. The resolution for mapping atomic motion is roughly 50 nm in space and 100 ns in time. By introducing auxiliary guided-mode (GM) fields that provide spatially varying AC Stark shifts, we have, to some degree, begun to control atomic trajectories, such as to enhance the flux into the central vacuum gap of the PCW at predetermined times and with known AC Stark shifts. Applications of these capabilities include enabling high fractional filling of optical trap sites within PCWs, calibration of optical fields within PCWs, and utilization of the time-dependent, optically dense atomic medium for novel nonlinear optical experiments.

INTRODUCTION: Headache is a very common phenomenon with a high economic and psychosocial impact. Tension-type headache (TTH) is the most prevalent (40%), especially amongst adult western women. The aim was to evaluate the existing evidence on the effectiveness of physiotherapy techniques in treating TTH. METHODS: Literature review of randomised clinical trials (RCT) and systematic reviews, published over the last five years, on the physiotherapy techniques most used in treating TTH: therapeutic exercise, suboccipital inhibition, cervical manip-ulation, massage, joint mobilisation and puncture. RESULTS: Twenty-six articles (seven reviews) met the criteria for inclusion. The reviews found evidence of the effectiveness of therapeutic exercise on the intensity, frequency and duration of pain. Improvement was also achieved by manual therapy in a similar way to medicines (although with contradictory long-term results), by the combination of dry puncture and physiotherapy on the VAS score, and by the combination of mobilisation techniques with stretching and muscular massage, but not separately. Amongst other results, the RCT showed that massage achieved less pain and frequency, as well as better quality of life, perceived clinical sensation and range of movement; pain improved with suboccipital inhibition and aerobic exercises; and the range of movement with cervical manipulation, massage of soft tissues and mobilisation. CONCLUSION: The published clinical evidence endorses physiotherapy as an effective treatment in managing patients with TTH, although additional studies with a better quality methodology are required.

BACKGROUND: To evaluate the effects on healthcare quality following implementation of a program to improve care for individuals with headache in two physical therapy clinics and its association with outcomes and self-perceived improvement. METHODS: We assessed healthcare quality by creating a questionnaire on fulfilment of fifteen quality criteria included in the clinical history of individuals suffering from tension-type, cervicogenic or migraine headaches seeking physical therapy between 2010 and 2014. In 2015, after applying a program to improve care in one center (C1), we reassessed the same fulfilment questionnaire in both centers, using the other center (C2) as control. RESULTS: In the first evaluation there was a huge number of cases of non-compliance of all the criteria in both centers. After implementation of the care improvement program in C1 a significant improvement was observed in some items, as use of a headache diary, which rose from 0 to 100%, or use of the HIT-6 disability questionnaire, which rose from 30 to 100%. In addition, there was a significant improvement in self-perceived health status after implementation of the care program in C1. CONCLUSIONS: The implementation of a care improvement program was effective in improving healthcare quality for individuals with headache attending physical therapy services.

Objetivou-se, com este estudo, avaliar o ajuste dos modelos de Brody, Gompertz, Logístico e Von Bertalanffy aos dados de altura na cernelha de equinos Mangalarga Marchador, ponderando pelo inverso da variância, a fim de selecionar o melhor modelo e predizer sobre o crescimento e a maturidade dos animais dessa raça. Foram utilizados dados de 230 equinos dos seis aos 176 meses de idade, os quais foram divididos por sexo e em 16 classes de idade. Os modelos estudados foram comparados segundo os avaliadores de qualidade: coeficiente de determinação (R2); desvio padrão residual (DPR) e critério de informação de Akaike (AIC). A estimação dos parâmetros dos modelos foi realizada pelo método de mínimos quadrados ponderado no software R. Todos os modelos avaliados se mostraram adequados para descrever a curva de crescimento em ambos os sexos. Perante os avaliadores utilizados, o modelo Logístico foi o mais adequado para descrever as curvas de crescimento em altura na cernelha nos dois sexos da raça Mangalarga Marchador. Observou-se também um crescimento acelerado nos primeiros meses de idade. Os animais machos atingiram uma altura adulta maior, porém as fêmeas são mais precoces, pois apresentaram maior estimativa para o índice de maturidade.(AU)


This work aimed to evaluate the fit of models Brody, Gompertz, Logistic and Von Bertalanffy to height at the withers of Mangalarga Marchador horses, weighed by the inverse of variance, in order to select the best model and predict growth and maturity of this breed pf animals. We used data of 230 equines from 06 to 176 months of age who were divided by sex and 16 age classes. The models were compared according to the quality assessors: coefficient of determination (R2); residual standard deviation (RSD) and Akaike information criteria (AIC). The estimation of the parameters from models was performed by the weighted least squares method in Software R. All models evaluated were suitable to describe the growth curve in both sexes. In view of the evaluators used, the logistic model was the most suitable to describe the growth curves in withers height in both sexes for the race Mangalarga Marchador. A strong growth at first age was also observed. The males reached a greater adult height, but females did so sooner, because they presented higher estimates for the maturity index.(AU)
